According to legends and folklore:

Once an entire kingdom collapsed because of one loose screw in the wheel of a chariot.
A decisive battle was taking place.
The general was inside a chariot.
The screw on the wheel of his chariot fell out.
So the wheel fell out.
So the chariot collapsed.
So the general died.
So the army lost their commander.
So the army lost morale.
So the army lost the war.
So the enemies killed the king.
And the enemies took over the kingdom.
The collapse of this entire kingdom was traced back to one person not properly tightening a screw!!

One little screw 🔩 !!!

A more recent example is a missing “hyphen” in the code of the spacecraft Mariner 1 in the year 1962. Because of this missing hyphen in the millions of lines of code, the spacecraft exploded shortly after takeoff and cost NASA $150 million in today’s money!
It was described as:
“the most expensive hyphen in history” by writer Arthur C. Clarke.
